The risk in the triangle shirtwaist fire was that there was a severe lack of fire safety, and the fire safety that was there which were mostly emergency doors were locked so workers wouldn't leave the building and the ladder on the side of the building litteraly fell off the side during the fire. You could mitigate them by not locking the emergency doors, doing fire drills, and having better matinence of the building
